Role Overview: The role is for a Bank staff who is an Advanced Assessment Therapist. Their responsibility would include completing community-based patient assessment with the aim to avoid unnecessary hospital admission. The role is to cover staff on a Career Break throughout the high service seasonal demand of the Winter pressure. This is fixed term for three months until end of March 2024.
The candidate is expected to have a valid UK permitted driving license with access to a vehicle for work purposes. The role will also involve supporting acute Trusts with discharge facilitation for service users referred via the D2A pathway. The candidate may be asked to support the Post Covid MDT clinic. During patient clinical assessments, the clinician is expected to be able to assess and identify the need for social care packages, medication concordance review, risk for clinical deterioration as well assess for any equipment required to enable the service user to remain safely in their home environment. The candidate may have to carry some of the equipment in their vehicle and is expected to utilize clinical expert knowledge and skills to identify a deteriorating patient then work on an escalation plan accordingly. Successful candidate is expected to be a team player, good advocate for the service users, be able to complete comprehensive documentation on clinical record system, feedback to MDT as well as liaising with other agencies involved in patient care.

 

Key Responsibilities:

  • Respond promptly to referrals from mental health crisis teams and other healthcare professionals, conducting rapid assessments of patients’ physical health and functional abilities.
  • Provide expert physiotherapy interventions, including assessment, treatment, and rehabilitation, to address acute physical health needs and prevent functional decline in patients experiencing mental health crises.
  • Collaborate closely with multidisciplinary teams, including psychiatrists, nurses, occupational therapists, and social workers, to ensure coordinated and holistic care for patients in crisis.
  • Develop personalized care plans and goals in collaboration with patients and their families, focusing on optimizing functional independence and promoting recovery.
  • Offer education and support to patients, families, and caregivers on self-management techniques, adaptive strategies, and community resources to facilitate successful discharge and follow-up care.
  • Maintain accurate and up-to-date clinical documentation in accordance with professional and regulatory standards.

Requirements:

  • Chartered Physiotherapist registered with the Health and Care Professions Council (HCPC).
  • Substantial experience working as a Band 7 Physiotherapist in rapid response or acute care settings, preferably within mental health crisis services.
  • Advanced clinical skills in conducting rapid assessments, managing acute physical health conditions, and implementing urgent interventions to stabilize patients in crisis.
  • Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and multidisciplinary teams under pressure.
  • Commitment to delivering high-quality, person-centered care and promoting recovery-oriented practice.
  • Flexibility to work shifts and respond to urgent referrals as part of a rapid response team.
