Job Title: Band 6 Occupational Therapist (Community Experience)

Position Overview: As a Band 6 Occupational Therapist with Community Experience, you will play a vital role in providing person-centered occupational therapy services to clients within the community setting. You will work collaboratively with individuals, families, and multidisciplinary teams to assess needs, develop tailored intervention plans, and promote holistic wellness and independence.

Key Responsibilities:

  • Conduct comprehensive assessments of clients’ physical, cognitive, and psychosocial needs within the community setting.
  • Collaborate with clients and their support networks to establish realistic goals and develop personalized intervention plans to address identified needs.
  • Provide evidence-based occupational therapy interventions to enhance clients’ functional abilities, facilitate community integration, and promote self-management skills.
  • Deliver education and support to clients, families, and caregivers on adaptive strategies, environmental modifications, and community resources to optimize independence and participation in daily activities.
  • Monitor clients’ progress, evaluate the effectiveness of interventions, and make necessary adjustments to intervention plans in accordance with best practice and individual goals.
  • Maintain accurate and timely documentation of assessments, interventions, and outcomes in compliance with professional standards and organizational policies.

Qualifications:

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited institution.
  • Current registration with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
  • Minimum of [insert years] years of experience working as an Occupational Therapist in a community-based setting.
  • Strong clinical skills in assessment, intervention, and evaluation across a range of client populations and conditions.
