Currently, we are seeking a Band 6 Occupational Therapist to join our esteemed Home First Discharge Team.

Job Description: We are looking for a dedicated and compassionate Band 6 Occupational Therapist to join our Home First Discharge Team. In this role, you will play a vital role in facilitating the safe and timely discharge of patients from hospital to their homes. You will work collaboratively with multidisciplinary teams to assess patients’ needs, develop tailored discharge plans, and implement interventions to support their transition back to independent living.

Key Responsibilities:

  • Conduct comprehensive assessments of patients’ functional abilities and home environments to identify barriers to discharge.
  • Collaborate with patients, their families, and healthcare professionals to develop individualized discharge plans focused on promoting independence and optimizing quality of life.
  • Coordinate community resources and support services to ensure seamless transitions and continuity of care post-discharge.
  • Provide hands-on intervention and rehabilitation to improve patients’ functional skills, mobility, and confidence in performing activities of daily living.
  • Educate patients and their caregivers on home safety, adaptive strategies, and self-management techniques to facilitate a successful transition home.
  • Liaise with community partners, including social services, housing agencies, and voluntary organizations, to address patients’ social and environmental needs.
  • Monitor and evaluate patient progress throughout the discharge process, adjusting interventions and plans as needed to achieve optimal outcomes.
  • Maintain accurate and timely documentation of assessments, interventions, and discharge plans in accordance with professional standards and regulatory requirements.

Requirements:

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited institution.
  • Current registration with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
  • Minimum of 2 years of post-qualification experience, with specific experience in discharge planning and community-based rehabilitation preferred.
  • Strong clinical assessment and therapeutic skills, with a focus on promoting independence and enhancing functional abilities.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.
  • Ability to work autonomously and collaboratively in a fast-paced environment, managing competing priorities and deadlines effectively.
  • Commitment to person-centered care, advocacy, and empowering individuals to achieve their goals.
  • Eligibility to work in the UK without sponsorship.
