Position Overview: We are currently seeking a passionate and experienced Occupational Therapist (Band 6) to join our team as a Community Rehab Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of individuals recovering from mental health challenges by providing comprehensive rehabilitation services within community settings.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals’ functional abilities and rehabilitation needs within a community setting.
  • Develop personalized treatment plans focused on promoting independence, enhancing quality of life, and facilitating community reintegration.
  • Provide individual and group therapy sessions aimed at improving daily living skills, social participation, and vocational engagement.
  • Collaborate closely with clients, their families, and multidisciplinary teams to ensure coordinated and holistic care delivery.
  • Utilize evidence-based interventions and outcome measures to monitor progress and adjust treatment plans as needed.
  • Participate in discharge planning and liaise with community resources to support seamless transitions and ongoing support for clients.

Requirements:

  • Bachelor’s or Master’s degree in Occupational Therapy.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Substantial experience working as an Occupational Therapist within a community rehabilitation or mental health setting.
  • Strong assessment, treatment planning, and intervention skills, with a focus on promoting recovery and independence.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and collaborate effectively with colleagues and external stakeholders.
  • Commitment to ongoing professional development and evidence-based practice.
  • Valid driver’s license and access to a vehicle for travel within the community (if applicable).
