Pulse is currently seeking multiple HCPC registered social workers to join an older people’s mental health liaison service in Northamptonshire. The successful applicant will be responsible for the assessment and treatment of Older Adult patients who are admitted onto the dementia and delirium pathway. 

The D&D service specialises in assisting with complex discharge planning for people with dementia and/or delirium who are acute hospital patients. The D&D model works closely with inpatient and community services to support patients and their families with the most appropriate discharge plan helping reduce delayed discharges and giving the patient the best chance of returning home.

This position is available immediately, offering full time hours for 5 months initially.
