Our client currently has an exciting new opportunity for a qualified social worker to join a well-established private practice based in Kent. The focus of this role will be to assess service users referred into the service by GP’s and other healthcare professionals, to determine mental health needs. 

You will be working within Tier 4 low-secure hospital for young people aged 12 – 18 with mental health and behavioural difficulties. This is a part-time role, working 15 hours per week. It is essential that candidates applying hold current HCPC registration. 

The role is available to start as soon as possible, offering full-time hours for an ongoing period.  

Skills and Experience needed: 

• Current HCPC registration 
• At least 2 years’ post qualification experience in mental health 
• Good assessment and care planning skills 
• Dip(SW), CQSW or equivalent qualification 

Your main duties and responsibilities may include: 
• Maintain up to date records utilising electronic media as necessary 
• To receive safeguarding referrals and undertake safeguarding assessments/investigations, following BCC polices and procedures 
• Assist in the process of rehabilitation and coordinate discharge planning arrangements 
• Standard case management duties 
• Finding out what type of care and support clients require 
• To undertake risk assessments per trust policy  
• To prioritise time and workload, in a flexible manner to meet community caseload responsibilities
