Our client currently has a requirement for an experienced social worker to join their well-established NHS trust based in Kent. You will be responsible for providing an initial response and triage referrals received from young people, their families and professionals based within the area. The purpose is to ensure that all referrals are triaged and assessed within the timescales and are appropriately signposted based on clinical needs and referred accordingly.

The ideal candidate will be graduate qualified health care professional and have substantial CAMHS clinical experience. You will also hold current HCPC registration and can work with young people with emotional wellbeing and mental health difficulties.

Your main duties will include, but are not limited to: 
• Maintain up to date records utilising electronic media as necessary 
• Assist in the process of rehabilitation and coordinate discharge planning arrangements 
• Standard case management duties 
• Finding out what type of care and support clients require 
• To undertake risk assessments per trust policy  
• To prioritise time and workload, in a flexible manner to meet community caseload responsibilities
