Are you a Care Coordinator looking to start your next role with amazing rates of pay?

Our client is now seeking a band 6 Early Intervention I n Psychosis Care Coordinator to join their well-established NHS trust based in South Yorkshire. The post is available to start as soon as possible, offering full time hours per week for an initial 3-month period.

It is essential that all applying candidates have previous experience of working within a healthcare organisation, to be considered for the role. It is also necessary that the appointed candidates holds a full UK driving licence as well as having access to your own vehicle as you will be required to travel between sites.

Daily tasks may include: 
• Developing care plans and rearranging them as necessary when difficulties arise
• Visiting patients, checking on the care they've received and documenting it accordingly
• Working with the care team to evaluate interventions and identify where and when further ones will be required
