Our client is now seeking a Band 6 Care Co-ordinator to join their well-established NHS trust based in High Wycombe. The post is available to start as soon as possible, offering full time hours per week for an initial 3-month period with the possibility of extension.

Previous experience of working with adults of working age or older, with mental health issues in a community setting and being a driver with access to your own vehicle are essential attributes for this role.

The pay rates shown are for Umbrella payment method. The PAYE equivalent is £21-£23 per hour. 

Daily tasks may include: 
• Developing care plans and rearranging them as necessary when difficulties arise
• Visiting patients, checking on the care they've received and documenting it accordingly
• Working with the care team to evaluate interventions and identify where and when further ones will be required
