£18,000 pa - plus bonus

Full time hours - 37.5 hour week to cover office open hours of 8am - 7pm (Mon, Tues, Thurs & Fri 8-4/9-5 and every other Weds 11-7)

Overview:

We are currently seeking a Patient Coordinator to join our National Diabetes Prevention Service team based within our Leeds City Centre offices. This lifestyle intervention is delivered by a team of Health and Wellbeing Coaches and Advisors and focuses on helping service users to reduce their risk of developing Type 2 Diabetes.

This role will provide telephone and administration support to our regional teams via the management of coaches through data implementation and telephone / email communication.

Job Scope:

Responsible for undertaking all administration duties required to ensure that patients receive a first-class service, with appointments booked in a timely manner. This includes:

  • Providing a first point of contact for all coaches and advisors registered to provide services through the National Diabetes Prevention Programme.
  • Ensure patients receive excellent customer service through the efficient booking of appointments.
  • Building relationships with coaches and advisors to ensure their continued availability.
  • Identify and escalate priority issues.

About you:

To become part of the Patient Coordination Team you must have an excellent telephone manner as much of this role is telephone based. You will deal with high call volumes and will need to have confidence in handling people with sensitivity and understanding. Strong organisational skills are also essential as you will be juggling multiple appointments at all times.

Background within a telephone/customer service based environment is essential, an interest in health would be an advantage although not eseential , as would strong IT skills given the technical aspects of the contract.
