Pulse is seeking to recruit a Clinical PWP Case Manager, to support clients with their health conditions and disabilities in a well-established private practice in Oakham, East Midlands. 

The Post Holder will conduct person centred clinical assessments to identify the needs of the client and to deliver brief clinical interventions and/or social prescribing in accordance with the client’s needs in a timely manner. You will be expected to deliver information, advice and brief interventions that are within the boundaries of their training, competence and professional code of conduct. 

Main duties and key responsibilities:

• Conduct thorough client assessments using investigative analytical skills and clinical reasoning, within set timeframes, identifying the situation, client needs, barriers and goals, including understanding their specific physical or mental health and its effects on their ability to work or to seek employment
• Use developed knowledge and experience to make a therapeutic diagnosis of a client’s condition and interpret clinical findings to develop an individually tailored Action Plan for each client which will involve a realistic time period for each activity against SMART objectives
• Where required to do so through service contracts, review and discuss the details of client cases with authorised personnel from commissioning agencies
• Manage the caseload and make contact with clients regularly to ensure each client feels supported and that optimum therapeutic outcomes are achieved.
