Are you a compassionate Occupational Therapist looking for a new opportunity to expand your skills?

 

Pulse are currently seeking a band 7 Lead Occupational Therapist to join our well-established NHS client’s trust based in Hertfordshire. The role is available to start as soon as possible, offering part-time hours per week for a 3 month period.

 

A band 6 physiotherapist or OT is required in the community therapy team to support the influx on priority referrals to the service as a direct result of a combination of The Discharge to Assess guidance and Home First drive over the Covid pandemic and the increased pressure from Primary care to avoid admissions where ever possible.

 

Specific skills:

 

  • The role of the In-Reach Team is to improve the use of bed-based rehab services across Hertfordshire. This is a multi-disciplinary team made up of clinicians and administrators.
  • The team will report daily on bed status and consequently, the role will be crucial to the flow of patients from secondary care to community beds and onwards. The Team Lead will manage the team of clinicians and administrators.
  • To enable safe and efficient transfer of patients into HCT bed bases by ensuring close relationships with social service, unit managers and the acute trust.
  • Co-ordinate good bed management through monitoring and planning. This involves liaising with discharge teams and other referrers to check that admissions are to the most appropriate location and bed type and ensuring that ‘trusted assessment’ protocols are followed. This will involve interaction with senior members of staff within Hertfordshire Community NHS Trust, the Acute Trusts and Hertfordshire County Council.
  • Suitability of patients to transfer to our community hospital.
  • Knowledge of bloods and understanding of what is safe for transfers and what needs medical review.
  • Knowledge and experience in rehab pathways available and directing to the correct one
  • Assessment and screening of all referrals including complex referrals
  • Shadowing experience will be given
  • System One Knowledge desirable
