Pulse Nursing are offering a variety of shifts for experienced Diabetes Specialist band 7 in Hounslow Our Diabetes Specialist Nurse provide care to clients with a variety of needs within their home environment throughout the community.

Pulse are looking for a enthusiastic Diabetes Specialist Nurse who will be responsible for providing specialist nursing support to primary care and community teams within the Hounslow Community Diabetes Service for adults with type 2 diabetes predominantly, and patients with type 1 diabetes in special circumstances.

available from Monday-Sunday with hourly pay rates £28.00 to £36.00 an Hour

Diabetes Specialist Nurse responsibilities includes:

The following tasks will fall under the purview of the chosen candidates:

• Participate in Multi-Disciplinary Group meetings, GP Education and case management, and virtual management of clients in the community.

• Contribute to the training and education of practise nurses, community nurse teams, and nursing home staff regarding the provision of individualised diabetes care.

• Visit general practitioners’ offices as part of a project to teach and educate primary care professionals and promote in-office clinics.

• Encourage patients to lead better lifestyles and manage their diabetes on their own, considering their unique requirements, medical conditions, and religious convictions.

• Assist in the implementation of service policies and evaluate how they will affect the provision of care.

• To improve the overall health of patients, work productively as a member of the multidisciplinary team that includes patients, specialist diabetes nurses, dietitians etc.

Diabetes Specialist Nurse Requirements:

  • Registered nurse with a current NMC pin
  • A minimum 6 Month NHS experience within the last 2 years
  • In date Driving license with use of own car
  • Meet our Framework Compliance requirement (we will guide you through this)
