An exciting opportunity for a Band 6 Occupational Therapist has become available within our well-established NHS Client’s team based in London. This position is to start ASAP, offering full-time hours per week with the placement due to end on 25.12.23.
The successful Occupational Therapist will be working as part of the Transfer of Care Hub facilitating and co-ordinating discharges from the Hospital. Duties include liaising with health and social care services to co-ordinate discharges from wards and also from step down bedded facilities. The postholder will be ordering equipment, advising on discharge, advising on home environment suitability for discharge as well as meeting with ward teams, social care teams and community teams/ step down bedded unit teams to help facilitate discharge. May require a home visits/ visits to bedded units. Applying therapists must have discharge experience from an acute hospital.
All candidates applying must have a valid HCPC registration, a relevant Occupational Therapy Degree/ Qualification, and previous experience in a similar setting to be considered for this role.
