Are you a compassionate Occupational Therapist looking for a new opportunity to expand your skills?
Pulse is seeking to recruit a Band 6 Occupational Therapist to join a community rapid response team at an NHS client based in Hampshire. The role is available to start as soon as possible, offering full-time hours for an ongoing period.
The successful Occupational Therapist will be responsible for carrying out rapid response assessment for crisis patients at risk of admission to hospital, facilitating urgent discharge from hospital. The ideal candidate will have understanding of clinical deterioration, baseline obs, MDT assessment, signposting, progression of rehab, caseload management and specialist equipment.
Duties within this role will include:
- Providing home care packages
- Carrying out risk assessments
- Making hospital referrals/discharge planning
- Devising rehabilitation programmes to develop skills
- Dealing with equipment to assist with daily living
- Managing own caseload
Skills and experience needed:
- Full licence and access to own vehicle
- Wide knowledge of long-term condition management
- HCPC registration
- Relevant degree/qualification
