Position Overview: Band 6 Community Rehab Physiotherapist

Are you a dynamic and skilled Physiotherapist with a passion for community rehabilitation? Pulse Mental Health is actively seeking a Band 6 Community Rehab Physiotherapist to join our team. In this role, you will be a key player in empowering individuals on their journey towards recovery and optimal well-being.

Responsibilities:

  1. Assessment and Treatment: Conduct thorough assessments of individuals’ physical health and create personalized treatment plans tailored to their rehabilitation needs.

  2. Community-Based Interventions: Provide hands-on physiotherapy interventions in community settings, fostering independence and enhancing daily functioning.

  3. Collaborative Care: Work closely with a multidisciplinary team, including occupational therapists, psychologists, and healthcare professionals, to ensure comprehensive and holistic care.

  4. Education and Support: Empower individuals and their families through education, offering guidance on self-management strategies and promoting a proactive approach to rehabilitation.

  5. Documentation and Reporting: Maintain accurate and timely documentation of assessments, treatment plans, and progress notes in compliance with professional standards and organizational requirements.

Qualifications:

 

  1. Registration with the health care professions council (HCPC) .
  2. Band 6 or equivalent experience in community rehabilitation.
  3. Strong interpersonal and communication skills.
  4. Commitment to evidence-based practice and continuous professional development.
