An opportunity has become available for a Band 4 Support Worker to provide risk assessment and management of patients in the community suffering with severe and enduring mental health problems. This role is based in Essex.
The successful candidate will start as soon as possible, working full time hours for an initial 12 week period.
The post holder carries responsibility for ensuring health and social care needs of patients are carried out in accordance with the patient’s care plan under the guidance and the recommendations of qualified staff for patients under the Recovery and Well Being and Assertive Outreach Community Teams. They will work regularly without direct supervision and be responsible for supervising other support staff whilst working as part of a multi-disciplinary team. In addition, they will work in close co-operation with other hospitals and primary health care based professionals.
In this role, you will be expected to:
- Contribute to physical therapist’s effectiveness by identifying patient care issues; providing information and commentary; recommending options and courses of action; implementing physical therapy directives.
- Assess patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying therapy histories.
- Assess abnormal conditions by reviewing physical therapist’s interpretations of patient evaluations and test results.
- Instruct and counsel patients by describing therapeutic regimens; giving normal growth and development information; promoting wellness and health maintenance.
- Provide continuity of care by developing and implementing patient management plans.
- Maintain safe and clean working environment by complying with procedures, rules, and regulations; adhering to infection-control policies and protocols.
