Pulse have a requirement in North London to cover two community hospitals for a Medicines Management Pharmacist. Ideally candidates will need to be experienced ward based pharmacist’s with good clinical and communication skills to deliver a safe and efficient medicines management service to rehabilitation wards in North Division.
Duties include medicines reconciliation , discharge prescriptions, ability to work independently and as part of a team, clinical screening, medication chart monitoring , possibly attend MDT meetings and ward rounds , patient counselling, advice to healthcare professionals, safe and secure handling of medicines. You will also need to work in partnership with rehab wards, falls therapists, community pharmacists, specialist consultants, nurses and an array of other healthcare professionals to provide best use of medicines and effectively manage risks associated with medicines use within the rehab setting.
Also , the following is mandatory: The pharmacist must adhere to our working practices which includes maintaining handovers, completing intervention recording, updating calendar whereabouts and collecting KPIs and recording daily work and collecting audit data .. Therefore it would only suit somebody who is confident with word and excel and can manage clinical duties as well as required paperwork.
This is a full time role covering 37.5 hours across 5 days (from Monday to Friday 9am to 5pm).
