PULSE@Home provides high quality complex care, for example clients needing long term home ventilation, enabling clients to live in their own homes.
Receiving one-to-one care and treatment in the comfort and convenience of home surroundings means you can enjoy full support from your family and friends. PULSE nursing and care professionals provide a confidential service at a time when privacy and sensitivity are so important.
The care provided depends on each client’s needs. Supported convalescence and rehabilitation after a hospital stay helps clients to make a full recovery. Where needed, we can continue treatment at home avoiding unnecessary journeys to hospital. We support people with chronic illnesses and fit around your personal routine – perhaps providing medication at home or in your work place. For short trips, perhaps a doctor’s visit, a PULSE companion can be on hand to help.

Our staff are thoroughly competent in supporting people at home with complex conditions such as:

We offer short or long term support for clients with a variety of needs including:
- Tracheostomy care
- Ventilation and breathing support
- Bowel management
- Catheter care
We can extend our services to provide and manage multidisciplinary packages focused on individual client needs - including physiotherapy, occupational therapy and psychiatry services.
How we work
Our professional, experienced and friendly team members are passionate about the services we offer. We meet each client at home and sometimes prior to leaving hospital to ensure that we deliver services safely and effectively. Where appropriate with relatives or friends taking part, we undertake a thorough assessment to ensure that we deliver care safely and effectively. We produce a holistic plan describing the care and support that the PULSE@Home team will provide.
The plan is totally individual and created for each client describing the care chosen to be delivered and the support that the PULSE@Home team will provide. Each package is tailored to the needs of each individual client and can include everything from daily or live-in care through to around the clock support. We can ensure rapid response home assessment where necessary, enabling early discharge from hospital and avoiding unnecessary re-admission. Our services include support for a specific condition through to complex packages.
Our clients
We plan in partnership with the different authorities and organisations that may be involved with arranging nursing and care services. These include continuing care teams, intermediate care teams and hospital discharge teams to ensure support is individual to each client and any transition from hospital to home is as smooth as possible.
All our clients can be assured the staff we provide to them are carefully selected to meet their needs; for each member of staff we place we consider their experience and skills.
