Position: ITU Community Nurse Co-ordinator (RGN)
Salary: £25,000 - £30,000 plus bonus & company benefits
Location: Leeds and surrounding areas
Hours: Full time, permanent (Mon to Fri - no shift work)

We have en exciting opportunity for a Community Nurse to join our team in Leeds.
Overseeing the clinical aspects of all care provision for our complex care clients, you will work to the highest level of standards encouraging best practice in all areas of work at all times.
You will be a registered Nurse with a current NMC pin number with at least 3 yeas post qualification experience. We are looking for a Nurse who has proven experience of working within an ITU setting, with extensive experience of ventilation care. Prior experience of working in the community is not essential as training can be provided.
Duties and Responsibilities:
·Undertake assessments for new packages of care
·Review care plans and risk assessments on a regular basis
·Assess competencies of the staff working in the packages
·Recruit any nurses required to join the team
·Coordinate and oversee care provision to complex care clients in their own homes.
·Manage the clinical aspects of the care in partnership with other healthcare professionals
Frequent travel is required to visit clients, therefore you must be a car driver and be flexible to travel to different locations around Yorkshire. This is a great opportunity for an experienced Nurse who is perhaps is looking for a change of direction to use their clinical knowledge.
You will have experience within an acute setting, such as ITU, HDU.
