Description

To clarify, we are seeking someone who is able to take on the role of assessing for TeleCare equipment. Prior experience of TeleCare is not required.

. The vision is to support people in the community to remain safe and independent in their own home. The Local Authority is actively working with housing and health partners to develop integrated TeleCare/TeleHealth services with the aid of varied funding streams.
·The strategy is for TeleCare/TeleHealth to become part of standard rehabilitative, care and support packages during 2009, as the multi-disciplinary teams across the local authority, health and housing gain the necessary knowledge and skills to maximise the benefits of TeleCare and imbed it into their mainstream services.
·To support this change process LBH are commissioning a number of dedicated posts to support the service areas to use TeleCare as an integrated part of their day to day activities.

Purpose of Job:

The postholder will contribute to developing the current standalone TeleCare pathway into an integrated function of assessment processes completed by health and social care staff across City

The post holder will be a member of the TeleCare Team, working across Community Services, Health and Housing and will:

·Be an ?agent of change?'; supporting health and social care staff to embrace TeleCare as part of mainstreamed services, ensuring that during 2009 the TeleCare Team can withdraw the majority of their support.
·Contribute to the development of policies, procedures and pathways for reviewing and changing the care providing specialist assessment and intervention solutions for service users with complex needs and those where initial TeleCare solutions are not providing the expected results. TeleCare solutions will always be evaluated within the whole package of equipment and services to appropriately meet the needs of the service user in the most cost effective way.
·Work closely with the care managers, therapy, nursing & housing teams to develop holistic care packages that include TeleCare solutions. In conjunction with the rest of the TeleCare team provide practical training and hands-on mentoring to these staff so that they are more able to incorporate TeleCare ongoing assessment, feedback and care plan changes into their therapy/care/support packages without specialist support.
